You can register for entry to USA via this site (http://www.visitusa.org.uk/visitors/esta.aspx). It removes the need to fill in the green form and will shortly become compulsory. One person can legitimately apply for others.

No not yet!
The new system won`t be compulsory until Jan 09. Up to then you carry on as normal with the green visa waiver form which you are given at check in before you board on your outward bound journey.

yes it removes the need to fill it in later, i just need my passport number to finish off the first part so i can carry on

absolutely
you can also have a retina scan at gatwick now too, which im going to do on our trip in a few short weeks ;):p015:
